Terms and Conditions

I/My child will be taking part in the 2024 Brooke Lee Modelling Graduation.
I/We have read and understand all the commitment, dates, times, and compulsory rehearsals I will need to be available to participate.
I/We acknowledge and agree to the payment of the $100.00 Graduation Fee, which is mandatory upon submitting this form, regardless of my child's participation in the concert.
I/We are aware that in the event my child withdraws from the concert any time from the first week of Term 4 (October 1st, 2024) up until Graduation Day, I will incur a re-pattern fee of $100.00.

Awards and Achievements

During Term 4 and during our 2024 Graduation Concert we mention our Students/Models Awards & Achievements for the year. Due to the number of accomplishments our Students/Models achieve throughout 12 Months within the Modelling Industry we would hate to miss anything. Please record any Modelling Awards/Achievements you or your child has achieved this year below. This can include travel, photoshoots, magazines, competitions, awards, applications, fashion parades, promotions etc. If anything, else is achieved between September and December please email admin@brookelee.com.au so I can record the extra Achievements down before Concert.
